Sharing your medical information

Your GP and pharmacy keep important information about your health. By giving consent, relevant medical information can be securely shared with other healthcare providers when needed. For example, if you visit the Huisartsenspoedpost or another pharmacy.

How sharing your medical information works

You give consent

Through MijnMitz, you decide which healthcare providers may make your medical information available.

Your information stays with your healthcare provider

Your medical information remains stored with your own GP, pharmacy or other healthcare provider. It is not stored in a central medical record.

A healthcare provider requests information

When necessary for your treatment, an authorised healthcare provider can securely request relevant medical information.

What would you like to do?

Manage my consent

Use MijnMitz to decide which healthcare providers may make your medical information available.

See when my information has been exchanged

Use Volgjezorg to see when your medical information has been exchanged via the LSP.

View my GP medical record

Use our patient portal to view information from your own GP medical record.

EXAMPLE

When you visit the Huisartsenspoedpost

Imagine you need urgent medical care in the evening, at night or during the weekend and visit the Huisartsenspoedpost. The GP there will usually not know you personally. If you have given consent, the GP can request relevant information from your GP medical record. This may include your current health problems, medication and allergies. This helps the GP provide you with appropriate and safe care.

More about sharing your medical information

Mitz is a national service where you can record your choices about sharing your medical information. You decide which healthcare providers may make your information available to other healthcare providers.

The National Exchange Point (LSP) is a secure network that allows healthcare providers to exchange medical information. The LSP does not contain your complete medical record: your information remains stored with your own healthcare providers.

Through Mitz, you give your consent. The LSP can then be used to securely exchange relevant medical information when needed for your treatment.

If you have given consent, another GP – for example at the Huisartsenpost – can request a summary of relevant information from your GP medical record.

This summary may include information about:

  • your current health problems;
  • your medication;
  • allergies and intolerances;
  • recent consultations with your GP.

This does not mean that another GP automatically has access to your complete GP medical record.

No. Giving consent does not mean that every healthcare provider can simply access your medical information.

Only authorised healthcare providers can request information when it is necessary for your treatment. Use of the LSP is logged. Your medical information remains stored with your own healthcare providers.

Through MijnMitz, you can use DigiD to specify which healthcare providers may make your medical information available.

You can review or change your choices at any time.

If you do not want certain information from your GP medical record to be shared, please discuss this with your GP.

In certain situations, information can be excluded from electronic exchange. Other healthcare providers will then be unable to request this information, including when you visit the Huisartsenpost.

Discuss with your GP what is possible and appropriate in your situation.

Through Volgjezorg, you can view information about the exchange of your medical information via the LSP.

You can see, among other things, which healthcare providers have made information available and when information has been requested via the LSP.

Your medical record is not stored centrally in the LSP. Your information remains with your own healthcare providers, such as your GP and pharmacy.

The LSP provides a secure connection that allows authorised healthcare providers to exchange relevant information. A healthcare provider may only request information when it is necessary for your treatment.

You remain in control: you decide whether healthcare providers may make your information available and you can change these choices at any time.
