Dear patients of Dr. Poelemeijer and Dr. Heimensem,
Through this message, we would like to inform you about a change in our team of general practitioners at Huisartsenpraktijk Vechtrijk.
Dr. Jolien Heimensem will unfortunately be leaving our practice on September 1, 2025.
She has cared for our patients with great dedication over the past few years. In the autumn, she will start at a GP practice in Dronten.
We thank her sincerely and wish her every success with this wonderful new step.
As of February 1, 2026, we welcome Dr. Els van der Veen as one of your permanent general practitioners in Weesp & Nigtevecht. Together with Dr. Youri Poelemeijer, she will form your permanent GP team. She is a familiar face in our practice: in recent years, she has enjoyed providing cover in both Weesp and Nigtevecht and looks forward to meeting you.
In the period from September 2025 through January 2026, we will do our best to work with permanent locums as much as possible. GP care will remain as you have come to expect from us: reliable, accessible, and personal.
We remain committed to providing high-quality care and a permanent GP for every patient.
Dr. Heimensem would like to explain her departure, and Dr. van der Veen would like to introduce herself as your new GP.
Kind regards,
General Practice Vechtrijk
Farewell from Dr. Heimensem:
After two years of being your GP with great pleasure, I will unfortunately be leaving Huisartsenpraktijk Vechtrijk as of September 1. It has not been an easy choice, as I have worked with great pleasure and dedication as your GP within a very wonderful team.
The reason for my departure is that I want to take an important step in my professional development: realizing my dream of becoming a practice owner. Unfortunately, this was not an option at Huisartsenpraktijk Vechtrijk, which is why I am moving to a practice where I can take on this challenge.
I would like to thank you very sincerely for your trust over the past few years. It was special for me to be allowed to be part of your life – sometimes in small moments, sometimes in very significant ones.
I wish you all the best and hope that you will continue to feel well-supported and at home in the practice in the future.
With warm regards,
Jolien Heimensem, GP
Introduction of Dr. van der Veen:
My name is Els van der Veen. I live in Hilversum with my family.
In 2021, I completed my GP training in The Hague. Prior to this training, I worked as a doctor in the Emergency Department and in the Internal Medicine department.
In recent years, I have worked as a locum GP at various locations in the region, including this practice. I am very much looking forward to starting here now as a permanent GP.
I enjoy the profession of general practice every day; we see people of all ages—from young to old—and often build a long-term therapeutic relationship. I believe it is important that we, as general practitioners, are easily approachable. For big and small problems.
I would be happy to work with you to find a solution to your health concern.
Kind regards,
Els van der Veen, GP
