From time to time, we will conduct a survey among our patients to assess satisfaction. In this way, we hope to continue improving and maintain our level of quality.
A complaint? Discuss your dissatisfaction with your GP
Our goal is to provide you with high-quality and personal care. Unfortunately, it can happen that things do not go as they should or that you are not satisfied with the care provided or the course of events. We would like to hear from you in such cases. This enables us to rectify any mistakes made and resolve misunderstandings. Additionally, your feedback gives us the opportunity to further improve our performance.
If you have a complaint about a GP, a practice nurse, a medical assistant, a locum GP, or about our way of working, we would appreciate it if you would take the time to contact us about this.
You can do this by filling out the complaint form below. We aim to process your complaint within 2 working days of receipt and will contact you by telephone. Your complaint will be handled carefully and confidentially in accordance with the practice’s complaints procedure.
If we cannot resolve the matter together, you can discuss your complaint with an independent and impartial complaints officer. The complaints officer will work with you to find a solution to your complaint. The officer can attempt to mediate. The complaints officer does not take sides and therefore does not pass judgment. Everything you tell the complaints officer is confidential.

Are you unable to reach a solution with the complaints officer?
If you have a complaint about (for example) the GP, the intention is that both parties initially resolve it together. If that is not successful, mediation by the complaints officer usually follows. This is often sufficient to resolve the complaint and restore the relationship between the GP and the patient. Unfortunately, it sometimes happens that this is not successful.
You have decided to continue your complaint procedure. From now on, your complaint is referred to as a dispute.

Stichting DokH
The GP practice is affiliated with Stichting DokH under the name of practice owner J.A.M. Clemens. You can contact them for complaints about the practice and/or, for example, the medical assistant.
